Insurance Information
 Capitol PT is a participating provider for most insurance plans in Wisconsin including Medicare. We also accept most private insurance plans, and workers’ compensation coverage. As a service to our patients, we pre-verify your insurance, file the necessary claims paperwork, and are available to assist with any step of the reimbursement process.
Capitol PT also accepts VISA/Mastercard for your convenience with covering co-payments or co-insurance payments.
